Commercial Aviation Grew Safer Despite Higher Fatalities in 2025

Commercial aviation reached unprecedented operational volumes in 2025, with scheduled passenger traffic rising to nearly 5 billion passengers across 38 million flights worldwide. Global safety assessments published by international regulators and industry monitoring organizations reveal a notable divergence: while overall accident frequency and fatal accident rates declined significantly, total onboard and ground fatalities increased due to a small number of high-casualty accidents.
According to the latest Safety Report released by the International Civil Aviation Organization (ICAO), the global accident rate improved to 2.2 accidents per million departures in 2025, a 13% decline from the 2.6 recorded in 2024. The total number of accidents involving commercial aircraft with a maximum takeoff mass (MTOM) exceeding 5,700 kg fell from 95 to 85 worldwide.
More notably, fatal accidents—defined as occurrences in which at least one person is killed—declined 60% year over year. Commercial aviation recorded just four fatal accidents in 2025, compared with 10 in 2024, reducing the global fatal accident rate from 0.27 to 0.11 per million departures.
The Catastrophic Event Paradox
Despite the decline in fatal accident frequency, overall fatalities increased substantially. ICAO reported that total deaths rose from 296 in 2024 to 387 in 2025, pushing the global fatality rate from 65 to 78 deaths per billion passengers carried.
Parallel data from the Aviation Safety Network (ASN), which tracks a broader range of operations, including non-scheduled flights and multi-use airspace incidents, recorded 548 fatalities in 2025, an increase of more than 30% from the 404 reported in 2024. TAlthough the ASN logged 156 accidents in 2025, down from 219 the previous year, the death toll was the highest since 2018, when 962 fatalities were recorded.
Safety analysts note that advances in modern aviation have largely eliminated systemic operational failures, shifting the industry's primary risk profile toward rare but catastrophic events.
"These results demonstrate that aviation can and must continue becoming safer, even as more people fly. They also reflect ICAO's leadership during decades of collaboration between States and the industry to identify risks, learn from accidents, strengthen oversight and improve safety worldwide," said Toshiyuki Onuma, President, ICAO Council.

Major Disasters Drove Annual Fatality Totals
The increase in fatalities during 2025 was largely driven by two catastrophic accidents, which together accounted for roughly 60% of all aviation deaths during the year.
- Washington Potomac Mid-Air Collision (Jan. 29, 2025): An American Eagle Bombardier CRJ-701ER regional jetliner operated by PSA Airlines collided on approach to Ronald Reagan National Airport (DCA) with a US Army Sikorsky UH-60L helicopter. The collision killed 67 passengers and crew aboard the regional jet and three military personnel aboard the helicopter.
- Air India Flight 171 Crash (June 2, 2025): An Air India Boeing 787-8 Dreamliner crashed shortly after takeoff from Sardar Vallabhbhai Patel International Airport in Ahmedabad, India. The accident killed 241 passengers and crew, leaving a single survivor, while 19 people on the ground also lost their lives, making it the deadliest commercial aviation accident since 2014.
Former airline captain and accident investigator John Cox, President and CEO, Safety Operating Systems, said the 2025 figures were "skewed because of the loss of a widebody aircraft," emphasizing that long-term aviation safety trends remain positive despite annual statistical anomalies.
High-Risk Categories and Fatality Breakdown
ICAO's safety strategy structures operational risk mitigation around five Global High-Risk Occurrence Categories (G-HRCs) identified within its Global Aviation Safety Plan (GASP):
- Loss of Control In-Flight (LOC-I): Aircraft control loss during flight, historically the leading cause of fatal accidents. In 2025, LOC-I concentrated nearly one-third (~33%) of fatal accidents within the high-risk categories, requiring sustained investments in advanced pilot training, stall recovery, and automated onboard flight envelope protections.
- Mid-Air Collision (MAC): Aerial collisions or near-collisions. Four MAC-related incidents occurred in 2025, causing nearly 15% of global aviation deaths. Managing airspace separation remains a top priority as commercial traffic expands alongside uncrewed aerial systems (drones) and Advanced Air Mobility (AAM) vehicles.
- Controlled Flight Into Terrain (CFIT): Unintentional impact of an airworthy aircraft with ground, water, or obstacles. Though CFIT registered only a single commercial accident in 2025, that single event generated over 10% of total worldwide fatalities, underscoring the severe lethality of terrain encounters.
- Runway Excursion (RE): Aircraft veering off or overrunning a runway during takeoff or landing. Seven runway excursions occurred in 2025, one resulting in fatal injuries.
- Runway Incursion (RI): Incorrect presence of an aircraft, vehicle, or person on an active runway. Commercial operations recorded zero runway incursion accidents in 2025, highlighting positive outcomes from airport communications, ground surveillance technologies, and adherence to the Global Runway Safety Action Plan.
- Other occurrence categories: Accounted for the remaining 42% of global fatalities.
Regulatory Audits and Regional Divergence: Mexico's Operational Challenges
ICAO's Universal Safety Oversight Audit Programme (USOAP) reported a global average Effective Implementation (EI) score of 69.8% among member states. However, considerable disparities remain between international safety benchmarks and national oversight capabilities.
In Mexico, statistics published by the Federal Civil Aviation Agency (AFAC) showed an opposite trend to global performance:

Industry analyst Fernando Gómez attributed the decline in medical oversight to operational bottlenecks and a chronic shortage of qualified medical and technical personnel within AFAC. Gómez warned that persistent staffing shortages could expose Mexico to another US Federal Aviation Administration (FAA) Category 2 downgrade, similar to the action taken in May 2021 after the FAA identified 28 safety oversight deficiencies.
For fiscal year 2026, Mexico's Ministry of Finance allocated MX$657 million (US$32.8 million) to AFAC, representing a 3.4% nominal increase over 2025. However, industry experts argue the increase may be insufficient to address the agency's staffing and technical capacity constraints.
Long-Term Regulatory Frameworks and Emerging Risks
To sustain the long-term decline in accident rates, ICAO's 2026–2028 Global Aviation Safety Plan establishes the strategic objective of achieving zero commercial aviation fatalities by 2030.
In addition to the five traditional high-risk occurrence categories, the updated framework introduces three emerging operational risks: clear-air turbulence, abnormal runway contact, and non-powerplant system component failures.
The report also highlights emerging hazards associated with lithium battery thermal runaway events. According to ASN, airlines continue to report dozens of cabin fires each year involving power banks, mobile phones, and electronic cigarettes.
"Data-driven findings are effective in encouraging States to focus on the areas where evidence identifies the greatest risks or gaps. ICAO remains fully committed to delivering targeted capacity-building, technical assistance, and implementation support to help States achieve measurable safety improvements," said Carlos Salazar, Secretary General, ICAO.
